Converting indd file to epub fixed layout. Issues with images and text

Hi, I am trying to convert an indd book to epub.  I have access to both CS5 and have downloaded the trial indesign cc 2017.  When I try to upload my file to amazon kindle publishing, my files looks like this:

Instead of this:

Some small differences.  The heading is an image and in the amazon version it now has a square border.  Also the font in the amazon version is not accurate. 

My hope is to convert my ind file exactly as it is into a kindle for Amazon & Lulu.  Any suggestions would be awesome.

You must use InDesign CC2015/2017, to create your ePub (not CS5). After creating it and successfully validating it, use the free Kindle  Previewer to convert the ePub to the mobi format: Kindle Previewer
