Using submit button to send the file name and save time in email

Hi,

I need your attention please

I have a PDF and on cover page I want to insert a Signature button, A text field (to insert title of document) and a submit button.

Once signed, the and pressing the submit button the document should be sent via email.

I need the recipient should receive the the document, with document name along with submit time appear in the mail..

The important bit is the code that sets the subject and body text of the email.
