Quiz feature keeps failing. What am I doing wrong?

I'm trying to help a client publish a very basic presentation. Just slides and a quiz.  It keeps crashing during publish, OR when the quiz comes up you can't click on it. Any idea what I'm doing wrong? I've spent a ton of time trying and rebuilding.

Its a single 5 question quiz with unlimited attempts on each question. It comes at the very end of the presentation. It shows all the feedback and won't let me

I created the quiz by clicking Manage quiz, then click add question.
