Yes, this is a known problem, alternately caused by and cured
by installing Service Pack 4 for Windows 2000. See

thread for a discussion of the issues and various workarounds.
Essentially, the problem is caused by installing one or more
security updates for Internet Explorer, which break the registry
entries for the HTML Help ActiveX control (hhctrl.ocx) -- the
component that is responsible for design-time controls like Related
Topics links.
