    Auto duplicating a recordset in a PDF

      Using Leopard 10.5.4 on a Mac with Acrobat Pro 8.1.2

      I'd like to be able to automatically duplicate a set of fields into which a user can add new information (each record should be unique) for a PDF document uploaded onto my website, http://kddidit.com/pdfFiles/ifIDieCurrentHealthCareProviders.pdf.

      The intention for this PDF file is for people to be able to download, fill it in & save/print it for their own use. As peoples' particular health history varies, I'd like to keep this

      I have a vague idea of javascript so explicit details would be most helpful.
