Have you updated the ACR plug-in to version 4.6?
In lightroom i have ACR 5.2
In photoshop 4.6
This bug don't show at any situation, there is several situations and you can work with it and do not see this.
Problem is in interpretation S and R pixels when we use recovery with high amount, if you have raw from S5 (maybe from S3) download example preset from above and test.
I scare to export my photos if this problem repeat in other situation.
Its lottery what you get when you export :|

Where i must report of this bug to resolve this problem?

Please post the raw file, or better yet a DNG file with embedded settings. Screenshots are basically useless without the original files.
You can use any photos from s5, if you want a upload more.

Yes, I'm looking into this. I think the 100% size rendering is probably correct. The preview size rendering seems to be quite inaccurate in this case.
When we can get update with resolve this bug?
The issue was that due to your extreme adjustments, the previews at less that 100% zoom size were not accurate. The 100% zoom size previews (and all converted/exported images) were accurate.
The next release will resolve the issue inaccurate previews at less that 100% zoom (they will more closely match the 100% size results).
