    How to get attachments and put into a Document

      I have a workflow where a user will be entering data into a form and the data will be displayed in a different format on a report. On the report I need to be able to put images (graphs) into the report.
      Is there a way to have the user attach the images while entering the data into the form, then embed the images in the report?
      What is a way to collect the images and get them into the report?

      Very new to Adobe LC, please advise.
