There seems to be a big problem with Acrobat 8 and Clip Notes. Do a search of the CS3 forums (up one level from here to get 'em all), and read the replies. I do not believe that Adobe has issued a fix for this on either the Premiere or Acrobat side.
I believe the problem is with Reader 9. Version 8 should work no problems, and in fact does for me using Windows Media. (I just don't like Quicktime, so I never use it.)

I had adobe reader 8.10? installed on my PC, and that gav me and client a black screen with ability to enter review comments. So uninstalled ver 8.10? and tried Reader 9 - I know you said it had problems but still had to try. That gave me the first frame of video instead of a black screen but then it crashed.
Next step is to uninstall reader 9 and install? just plain reader 8? I read somewhere that the 8.10? version was buggy.
fyi, Clip notes works perfectly for my other client and my partner with whom I tested so I know the problem isn't on the sending side.
We need to get this finished asap, so I've resorted to sending .mov files to this client but will relook at the clip notes problem when have more time.
