Could this be a subtle (or not so subtle) hint that the Axio should not be used? Ask Matrox.
I'm very confused by your description of the problem.

It shows what is displayed on the two windows where the Source and Program monitors are. It should be showing Color Bars.
I agree with Harm; ask Matrox/Matrox forums. Since no one has jumped in with "known issue," I'm not confident you're going to solve it here.
That said, as Jim, I am confused by your description - but I don't have any fixes for any of the scenarios I can imagine either! The only similar symptoms I have seen (no matrox) appear to be video card related (source/program monitor display diagonals instead of correct pix).
The confusion: are the color bars what you have on your timeline? You're just using those instead of video for convenience in displaying the problem, right?
You refer to an "external monitor" and a "second monitor." Are those the same monitor? What is the external monitor - a monitor or TV?
I'll have John Concannon post back (he is the one that does the editing/uses the software). I have no previous experience with Adobe Premiere...so when I typed out that first post...I was just going off what he told me via phone...
...and from what I gather:
The external monitor is the 42 inch TV monitor.
The second monitor is the extension of the desktop.
Definitely have John post a description. The pic helps, but more details about how he got there and what he's tried will also help, and that's the part of your original post I found confusing.
