There are no fields which you can fill with Adobe Reader.

You're confusing "enabled" with "allowed".
If I were to guess I'd say you are looking at the security in File>Properties and seeing the word "allowed".
That just means if the creator of the file had added form fields, you would be "allowed" to fill them out.

I've got an older full version of Acrobat (5)- am I able to in some way take this document as a template and then create my own output, allowing me to generate a digital document?
If you have Acrobat 5 and not Acrobat Reader 5 (they've since took Acrobat out of the name and it's now only called Reader), you can add form fields that you can fill in with the form tools.
SO ijust open the doc in Acorbat 5, drop form fields into the document and away I run?

No. You have a fillable form. You can fill in the blanks using Reader.
