Use Compound Blur or Lensblur (or any other blur effect that supports this) with a respective greyscale input. Tilt shift is merely a trickery by changing the focal plane in the camera which you can fake this way in post. If you search the web, you can find more inspiration and examples easily. The trick is of course, that for this to work the imagery should be perfectly sharp to begin with. Additional "flattening" by using Corner Pin and similar effects for perspective correction may also improve the illusion.
