If you're loading in the FLV from outside the SWF using a relative link then make sure the HTML and the SWF file are in the same directory. When you load the SWF into the HTML, the SWF will attempt to load relative links from the starting point of the HTML file. You can also change the path of FLV, but it may not load outside of the HTML in that case.
There are also security restrictions that apply to a webpage that don't apply to the Flash authoring environment. If the FLV is loaded from another domain, it may work in the test environment but not in an HTML file or even the stand-alone Flash Player.
