My only thought was to update the printer drivers:
HP Photosmart Full Feature Software and Drivers - Mac OS X v10.3.9, v10.4 and v10.5 01-2008 9.7 - 189.53M
If you have the current version, try uninstalling/reinstalling them to see if it helps this issue.

Can you post the form somewhere for us to look at. It could be a pdf file problem, it could be a driver issue. Hard to tell without looking at the file.
Following on from my earlier problem I have tried the following
solutions with varying levels of success.
1. I loaded both Adobe Reader 9 and the document in question into
another computer, a PC running Windows 2000. It all worked perfectly
and printed as rquired.
2. I then went back to the Mac and reinstalled both Adobe Reader 9 and
the printer drivers and got the same result as before, i.e. no printing
I then loaded the attached document using Adobe Reader 9 and
attempted to print it again with the same result, that is
When I attempt to print I get a dialog Box
"The document could not be printed"
followed by second dialog box
"There were no pages selected to be printed"
For some reason the Mac does not want to print any Adobe document
