I just uploaded a file that may help.
Expand the file and inside the zip is a swf named "clicksound". Insert that SWF as an animation onto the question slide. Size and layer it so it covers your buttons.
Seems to work here.
Please give this a try and keep us posted on if or how well it worked.

I saved the file to the desktop and ...insert > animation, which put it on the slide. No change. I probably need to do something else? I included a screen shot.

You missed a step.

Your screen shot shows that you just plopped it on the slide and somehow expected it to work. It works by providing a layer that sits above your buttons. In operation, when you click one of the buttons you are also clicking the layer above (which triggers the audio in the SWF).

The clicksound.swf works a little too well. It randomly clicks twice. I do you have any suggestions? I have attached some files and a screen shot.

Hopefully you aren't using that SWF for anything other than buttons on the Question Slides. The intent was that since these buttons don't offer the property to control a click sound to provide one.
If you are perhaps placing the SWF over a typical Button or Click Box, you are likely hearing the mouse click sound normally produced in addition to the one inside that SWF.

I put the swf on a non questioin slide for demo but it works the same on the question slide.. I have included my quiz file this time. Correct answers have a period in front of the question.
