No promises, but it's worth a shot.

You may need to select More Options first.

Yes, I do have 'Include Book Documents (bookName)' ticked and it is showing the correct book name. Definitely worth checking for the obvious mistake

*That is to say: the InDesign Interchange format export-import tango did not help.

To make InDesign include all my text in the ToC, I had to open each chapter, cut all the text out of the threaded text frames, delete the text frames, install a new set of threaded text frames, paste the text in to the new text frames.
