When you have finished editing, you will go to Export>Adobe Media Encoder (AME). There, you can set your Export settings, load the Queue and then Export.

...leads me to the Export Settings window. Every choice I make shows an .mp4 extension. I cannot find any way to change that to .mov.

You should have a drop-down for QuickTime and can choose MOV, and then the Compression = CODEC.
I'll be that it's just in a drop-down, that you have not found yet.
Now I see where I went wrong...
The requirement was for H264. There is an H264 in that first drop down where QT is and I chose that. What I didn't realize is there is a second choice for H264 in the Video Codec selection further down.
But I did loose the readout for file size when changing to these settings. Wonder why?
Glad that the "look" helped, and sorry that I could not do it specifically for CS4's AME, or for the Mac. Still, enough similarities to get you started.
As for the loss of the settings, that might be a function of changes in CS4's AME. Someone else can probably point to to what needs to be done there.
