    How to get user feedback

My organization publishes many pdf documents for a world wide user base.

I'm looking for some help in how i may be able to obtain 'real-time' feedback from users on

comments/concerns, or errors they may find in our various documentation.

Is there a means to provide some sort of BUTTON on every page of a pdf document, that

user can click that may provide a comment pop-up?

I'd like to be able to capture:  Comment, page number and document name and compile

in some sort of database, or file, for sorting.

Does anyone have any ideas on how best to do this?
