What issues are you having? Do you not see the itemIndex property (get a compiler error when you try to use it) or is it not returning the right value for you when you do use it?
Like the data property, itemIndex is set by the DataGroup when creating the item renderer. It will probably start out as -1 or some other invalid value before being set correctly.

The issue at hand is not the value of itemIndex, I can see it if I put a breakpoint in my code but more it seems the GETTER method does not seem to pass the compiler.
I did a few tests this morning and it seems to relate to the issues I am having with the latest milestone SDK and using a Flex Library project:
it appears the Library project is still using some older classes of SDK????
I guess this question is answered then (Thanks guys) though I still have the problems with the Library which are doing my head in.
This was an issue with SDK's and not itemIndex, I do apologise for the inconvenience.
