    Creating a questionnaire

      I'm redesigning a site and need to include some online questionnaires with results feedback into a webpage- ie a total score, a prepared comment depending on the value of that score and a response to each answer:




      At the moment I'm trying to do it by cutting and pasting code from the original pages into my new layout but what with the fact that the original files on the server are all over the place and its clogged with irrelevant and redundant files plus the fact that I also don't know the purpose of some files and how I should alter them, it's not working.


      I'd be better off building my own from scratch so it's something I can do again in the future. I'm a designer not a programmer and I've got no grasp of php. Can someone point me to good and simple resource to create such a questionnaire - referably one which takes the trouble to explain the operation. Also any recommended video tutorials.

          The thing is there are pre-made solutions out there in PHP and hosted solutions such as Wufoo, but looking at your description and the questionnaire it sounds as though the quiz is a custom format that would require programming at some point.  If I were in your position I would look to outsource the coding of the questionnaire unless you are willing to learn PHP programming to modify a script to meet your needs.
