For most, just choosing Window>Workspace, and either choosing an existing Custom, or resetting to the Default, has cleared this up.

CS3 had a problem called title creep where the tools would start moving off screen.
Doing what is suggested above will reset it to the correct positions.
You can then do a work around to stop it from creeping.

Read Rogers Post and the associated thread for more info on this matter: I think I may have found where the problem lies. It appears that the "Title Tools" panel is the one that moves, pushing the position of the others. I have found that if you change the position on the Title Tools panel all the way to the bottom (of the titler window) the problem goes away (at least so far in my testing).
I know this fixed it for me with CS3
try draging the windows to different spots till you find what you like.

Great news! I have "lost" my tools too. In each case, I attribute it to OE on my part - just an errant click, or similar. A reset of Workspace has always cleared it up for me. Glenn does point out an issue that cropped up with CS3. I do not recall hearing of it in CS4, so maybe whatever the cause, things were sorted out with the upgrade.
I strongly recommend that every user create a custom Workspace, and maybe one for different operations. I took the default set and customized them for my dual monitors, saving each with a unique name.
