Select the outside pth nd the two inside paths and make the compund path.
All three have to part of the equation.

I am not quite sure what you mean by "lower inside hole in the B" but I have tried all the possible combinations of selection and attributes that you describe and it only filled the hole or had no effect
I have uploaded the ai file if anyone feels they are able to understand the problem in more detail.
Actually for some reason I did not focus on the Red circle you madeto show the problem.
You are using the wrong tools for this what you want to do is to turn your art into a Live Paint Group which you can always add shapes to in the future.
Select all your art that you want be part of the group click on the live paint bucket fill the tansparent parts with a fill of none.
You can expand it if you wish. It actually kind of turns it into a compound path.
It is the easier way to do this the art that does not fill work properly would otherwise have to be a closed path with the extended lines as separate paths.
if you turn off the fill group in the layers panel you get this
I discovered you can duplicate the fill group in the layers pane fill it with another color and and turn them on and off. which is handy as well
Here I have a yellow over a green with some transparency so it works like the appearance panel
