Do you have access to the script to modify such things?
If not, and this is a pre-made solution from your host that you cannot edit, you may need to contact them for the answer.
If it is something that is handled at the host level, then why does the subject work on one and not the others?
When I was using a much less sophisticated program (Web Easy) both the subject line and the email always came through.

But first, make sure your subject field is defined correctly.
Just a wild guess...if you gave a link to the working form I could check this....should the names of the email and subject fields not be capitalized? Check the working form and literally copy and paste them over the fields in the non-working form and see if there are any (even capitalization) differences.
