Scroll down to the final few posts on that page the rest at the top is what you have said that you went through. The only suggestion I have is that you consider migrating away from ADDT because it is a discontinued product and Adobe has stated on their development page that they will be merging the roadmap with DW for future versions of the software. From what I can tell if you uninstall this extension you should trash all the preferences/configuration files (depending on OS) and then restart DW. If the corruption is beyond that then you could consider the CS4 Clean Tool and re-installing DW (backing up first of course). That should only be a last resort though.
So get this. After disabling, uninstalling, reinstalling both the extensions and Dreamweaver. Even deleted some new printer drivers because that was the only other thing that had changed, it came down to this...the DOCTYPE!
Not exactly sure why, but the DOCTYPE on my page had changed from
<!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN" "http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd">
<!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN">
That's what broke the extension. Apparently ADDT needs "http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd"! As soon as I added it back in, it worked without any issues.
