          <form method="POST" action=""

          The action attribute of the form tag is empty.

          The form must be submitted to a server side script or else nothing is going
          to happen.

          All that is there now is a bunch of client-side javascript validation.

          What does your host's FAQ/support pages say about form to email scripts?
          Is there a control panel or CPanel? look around in there.

          If no pre-installed form to email script, what flavor of server side
          scripting does your hosting support?
