It is not clear what you are after. If you want to make a form available for download then create the pdf version of it and a link to it in the web page. If the intention is to have an online form, then create the form using html code elements and save it as a web page and provide a link to that wherever you plan for the link to be.
Thank you for your reply. I guess I wasn't very clear.
I have created a PDF and now I want to PUT the PDF on the server and create a link. The problem is, Dreamweaver doesn't recognise the PDF so I can't save it to the remote server using Dreamweaver.
Just to be sure, I just took a pdf file and using file manager software (My Computer, etc) placed it in a folder of a web site I have set up for DW. I opened DW and was able to Put that file on the site's server with one click. If that doesn't work for you, then I have no explanation.

I'm clearly missing a step somewhere. Here's what I'm doing.
I create a PDF with input boxes. I save it in the local sites root
directory. Then I go to DW and try to access the file but the actual PDF
isn't visible. Do I have to save the file as something else?

Try refreshing the site cache. Any file in your site should be visible in file manager.
