I've got exatly same problem on my Vista machine... Reinstall it, then install it back, no luck...
same thing here on Win 7 64 bit. Called tech support and after 3 hrs on hold was told there was no phone support for this product.
did a fresh install of windows installed only FB4 and CFB and got same. error is repeatable every time.
after I get the error 16 both FB and CFB get stuck in endless accept license loop. and will not run.
Lack of support is un Acceptable!!!!!!! for $700 Software.
Made them refund my $699 now waiting 10 days to get my money back.
The Price for being good sheep and buying on day 1.
Can you check the permissions on the directory
/Library/Application Support/Adobe/Adobe PCD/cache/
and the file
/Library/Application Support/Adobe/Adobe PCD/cache/cache.dbAlso, are there any files in that directory that end with .journal?
Same question for the folks using Windows. The directory on Windows is
C:\Program Files\Common Files\Adobe\Adobe PCD\cache
for 32-bit Windows
C:\Program Files (x86)\Common Files\Adobe\Adobe PCD\cachefor 64-bit Windows.

I checked the file permissions and they were fine, so I removed the cache.db file. I then relaunched Flash Builder 4 and it then ran fine.
I had to reactivate Flash though, but other than that, all seems to back to normal :-)
I wouldn't recommend deleting the cache.db file as it will also require you to re-enter the serial number for existing CS3 and CS4 apps, but I am glad it solved your problem.
