I find that I have to dump cookies and cache as well if I make a change to a favicon, which is why I've gone to using animated GIFs for site icons. I use the file path to the images folder instead of just a reference to the icon, and if I update a GIF, it updates immediately.
all you have to do is to overwrite the old icon file, nothing else.
If you have bookmarked the site in your IE, re-create the bookmark.
Or simply check with another browser, the new icon will be there.
Clear ALL your caches!
If you previously had no favicon to your page, your browser must be convinced first of them to look again.
Has proven useful:
- If set, delete the bookmark to the page
- Quit browser
- Empty the Temp Internet Files Internet Options
- Clear History
- Delete everything in general, where the browser remembers visited sites.
- Clear Site visit
- Clear Bookmark

I can only get the favicon to work on my home page, all other pages have identical code at the headers but don't show the favicon.
I use this code (highlighted in Bold):

First it is very bad manners to jump in on another thread this way.
But to answer your question the favicon should be place in the root directory not in the assets folder.
What I was taught was;
If you're using a .ico for your favicon, it MUST be in the root folder and ref'd there.
If you're using a GIF or PNG for the favicon it can go in your images folder as long as it's ref'd there.
I had tons of problems with ico files and DW cache holding the old image. I'd change an ico in one site and DW would show the new ico as the favicon in every site on my system. Even in previews the new icon would load with every page in every site, on Win and Mac. It's a real pain to have to clear that cache every day, so I quit using ico files altogether.
I use this code:

it points to an animated gif and it displays as the icon in every page with that code in the head.
