I am currently chewing through much of the provided material and it has taken me places that I have never been before.
I still cannot find one key component in my question:
How do I make the layer (OCG) visible ONLY after all the check boxes are checked.
If 3 out of 4 of the defined check boxes are checked, I do not want the OCG to be visible.
Only after all 4 check boxes are checked do I want the OCG to become visible.
I would really appreciate your assistance!

You need to use the logical operator AND, which is: &&
This is a form scripting issue, not really an OCG issue. First, you need a
document level function that checks the value of all the check boxes and
decides whether or not to enable the OCG. Then you need call this function
from the MouseUp event of each check box.

There are several ways you could implement the script for counting the
number of check boxes checked. This is one way.
GetMyOCG().state = this.getField("Check1").isBoxChecked(0)
You'll have to write the code for getting the OCG.
