i'll bump this as well if you don't mind - i have same problem.
running Reader 9.3.2 on XP SP3.
no matter what I try it always opens with Select tool active, rather than my preferred option of Hand tool. Every startup I must manually change it to Hand tool. Very frustrating. I'd tried changing many options in the preferences but no joy so far.
Any solution/suggestions would be great, thanks.

I changed some of the default preferences and this seems to have corrected the issue.
(Run adobe reader as administrator)
I changed the following settings:
Restore last view >> Check
View PDF/A mode >> Never
Don't know if this is what fixed it, or merely running as administrator.
Here's the only solution I've found to work. I don't know if you can do this in Acrobat Reader, but in Acrobat Pro 9 (WinXP), I open the Pages panel, right-click the first page thumbnail, select Page Properties, select the Actions tab, leave the Trigger at "Open Page", click the Add button (to Execute a menu item), select "Tools>Select & Zoom>Hand Tool", OK, Save. Now whenever you open that file the Hand Tool is the active tool.
Here is what I found.... Go to:
In the section marked "Other Accessibility Options", de-select the check box labeled "Always display the keyboard selection cursor"
This fixed it for me.
