You put your picture/image into DW and then you will see that:
Use the forms as you want/need and put it over the parts of your image and connect them with your links.
You should look at using CSS to do this website. It is an easy "two column with header (and footer if needed). You could use background images for the left column and put your links over it that way. Search for that, it's really not that hard.
To remove the lines in your tables, you need to set the cellspacing="0" and border="0".

You could set the image in the table column on the left to be a background image as below:

It would be best to set the width of the <td> to the width of the image or a little less. You can add your links on top of the image if you do it that way.
I still recommend using CSS to design the site though.

For equal height CSS columns, use your graphics editor to make a 2-color background image the same width as your page and about 20-50 pixels high. See 2-col background image below:
With CSS, apply the background image to your #container division and repeat-y (vertically) down the page. This techinique is known as Faux (fake) Columns.
