(Other than that one might possibly be able to automate the task with Actions or Scripts.)
2) No. And I for one am quite content with this behaviour.
Edit: Someone was trying to set up a Scripting-solution that would store the position of the original files in the metadata to be later on used to replace the content automatically; but I don’t know if those plans ever came to fruition and I can’t locate the thread at the moment.

So if the base is 4 x 6 at 150 ppi and the placed smart object is 2 x 3 at 300 ppi - IOW the same pixel dimensions - the smart object will still come in filling exactly one quadrant.
I don't know of any way to make smart objects honor pixel dimensions. But then of course you could just copy/paste and then convert to smart object.
