You assigned the db column to be Tiny Int, and from the value="sms" you are trying to insert text. The db will not allow that, if you want to insert text change to Varchar and assign a maxlength, or change the radio to input a number. You can use phpmyAdmin or the MySQL command line utility, which everone you used to create the db with.
ALTER TABLE your_table_name CHANGE your_column_name your_column_name varchar 50;
will change to a varchar with 50 maxlength
I don't want the column to have any text, I just want t to show if the check box was ticked or not. Do I still need to change it to Varchar?
All you need to do then is change the value of the radio button to a number
<input name="sms" type="checkbox" value="sms"> value is text
<input name="sms" type="checkbox" value="1"> int value
