With the help of my brain and a hint in another post I've been able to solve the problem.
What was the troublemaker ... I'll never know, but deleting the folder "C:\Users\Sam\AppData\Roaming\Adobe\Premiere Pro" was the solution.
I have found the culprit file!!!
Its [Effect_Presets_and_Custom_Items.PRFPSET] located in
Delete that and restart.
Apparently the newer Premiere builds dont like new projects to be made and loaded up after previous custom presets are made.
I have noticed it only does this on projects I have made AFTER creating presets.
And I am on CS6

Thanks - I didn't want to remove all my custom presets, but it was the pointer I needed.
For me the problem was a specific preset, the last one I added. Maybe I crashed out of that session, or somehow it got corrupted. Anyway, just deleting that one custom preset inside Premiere fixed it for me.
(CS6 / Win 8.1 x64)
It's also possible creating/deleting _any_ preset clears this up, as it may rewrite the preset file. Something to try ...
