The mode can be anything, the hotkeys don't work just after launch.
If the menu bar or tools bar gets the focus once, then the hotkey starts working.
e.g. Right-click on toolbar or activate a menu on the menu bar, then Ctrl+plus/Ctrl+minus starts working.
I'm running Windows XP with SP3.

I've been having the same issue for months. Why isn't this fixed, it's very annoying.
I had this problem before and it now appears to be fixed for me. Win 7 64 bit. Adobe X version 10.1.0. I don't know exactly when it was resolved.
CTRL-minus, CTRL-plus and CTRL-0 all work for me without needing to give focus to the menu bar or tool bar.
Check that you have the latest updates.
