The PLUS subscription allows you to create unlimited forms and you can use one account to create forms for all your customers. Here are answers to your other questions:

>> As part of embed we will allow for basic customization, however, support for CSS styling will be added in the future

>> We are working on adding basic notifications which will allow you to forward every response to email addresses you specify.

>> Today we allow form authors to customize confirmation page. Are you looking for something more?

>> We will add this feature to our product roadmap

Thank you so much for your response. This service sounds very good. For CSS, I generally like to be able to set the font face, size, and color as well as size of form fields. For the forwarding page, I often send visitors to a static page that I've already created (ie thankyou.html). I forgot to ask about forms to collect credit card info. Can I do that too? I have a few customers that are non profits who have fundraising events a few times a year and need to collect payments.Do your forms integrate with PayPal accounts?
Hopefully you will be able to customize font type, size, color, and field size in the FormsCentral design application itself and not need CSS styling. Ability to support third party payment gateways, including PayPal is high on our roadmap but it might be a while before we get to it.

Just wanted to give you a quick update. We just added the ability to embed the form into a web page. Just go to the distribute tab and click the Embed button. Please try it out and give us your feedback.

Glad that my suggestions are helping make FormsCentral even better. I feel terrible, though, as I have yet to log in and test it due to work obligations. My hopes are that I can finish up some projects in the next few months, after which my aim will be to play around with it.
FormsCentral now provides the ability to integrate your form with PayPal. In one seamless experience, respondents can fill out the form and pay using a credit card or PayPal account.
