It sounds like you've been creative in trying to select and delete from as many options as you can find.
But it worries me that you had a slew of these. I think that makes it more likely that there is other corruption.
It also worries me that you were able to delete buttons, etc; the phantom menu problem always seemed like you couldn't quite get it selected to delete it. I may be wrong about that.
I would also consider this suggestion from Jeff to try deleting the AuthorScriptHDMVSessions folder.

zorrocbr Feb 28, 2011 11:43 AM (in response to Stan Jones)I was only able to get to the Phantom Menu in Encore after clicking on one of the "missing links" that came up in the check project stage. Once I did that the menu showed up in the menu drop down list but not in the asset list. It gets better. So I went to the last page of my good pop up menu and added a right button next page arrow and then linked this to the Phantom Menu. On the Phantom Menu I added a buttom for Main Menu to link me back to the Top menu just in case somehow I got to the phantom menu when playing the disk and got trapped there with no hope for escape. I built the disk, it plays back fine. I went to the last page of my pop up menu and clicked on the next page button..... and NOTHING. It never went to the Phantom menu. (Good results for me because I didn't want it anyway, but just WEIRD).

There are many similar threads, and many ways that users have been able to work, when this happens, plus discussions on how it can happen. Some go back to before BD, but some include BD discussions.
