"scanned image in PDF" You have a picture/image - not renderable text. So, no table structure or text content to export to RTF.
How did you create a PDF if you only have Reader? If you have Acrobat then you can OCR the file, which will allow you to copy the text to
But let me try and understand. When I scan, and choose option Scan as document/PDF. Still the .pdf file I get is image? I can open perfectly in the reader...

Sounds like your scanning software has the ability to save as a PDF.
Still, it is a scan, therefore just an image. You would need to run what's called OCR on the PDF first to create live text from the image of text, then you would need something that could edit that text like Adobe Acrobat (probably not the best choice but there really aren't any good editing choices for PDF) or you could try to export it out of Acrobat (after OCR) into an editing application like Word and such.
But with Adobe Reader, you can't do anything.
