Twisties/Hotspot are supposed to work as effects in expanding/dropdown HotSpot working in online help format like WebHelp/Flashhelp etc. In Printed Doc output these twisties/dropdown HotSpot don't work as an effect rather the embedded text is included/excluded as per the settings done in Printed Doc output.
To include/exclude the text for Expanding/Dropdown Hotspot you may go to properties dialog of printed Doc SSL, click Advanced and check/uncheck corresponding settings in the dialog.

Are you talking about the new Review features in RoboHelp 9 rather than generating printed documentation?
If so you cannot include the dropdowns / expanding text in the review. This is a known problem. You might want to create a feature request for this.
