However, it doesn't on 11.0.3 running Mac OSX.

Therefore, there must be another location that contains the code that sets the style for printing?

The code on the right (in Print Preview) is clearly larger than the code on the left (Code View), which I can see clearly.
What are you trying to achieve? Do you want the code smaller? Bigger? The same?
Note that most Print Previews are not scaled accurately. That is, you can't reasonably compare what you see on the screen to it.
Also, many printers have options to "scale to fit paper" (or something to that effect) which is enabled by default. You might try finding and disabling this setting and see if that would affect your output.

I would like the code print out font to be a smaller size. Currently, it ls printing at 12 pt Monaco ( I can emulate this exactly by printing the same document in an editor set as Monaco 12pt ). However, the settings (see below) are set at Monanco 9pt. This works on the screen, but not for printing.

Yet the code:print gives me more characters per line, resulting in many more pages per printout. The "print preview" shown in my last message is exactly what the printed page looks like as well.
NOTE: with the same settings ("smallest Monaco 9pt), Dreamweaver CS3 on OSX *does* code:print the smaller size, though without color coding. Dreamweaver CS 5 on my system does not.

I just had the exact same problem on OSX with Dreamweaver CS 5.
Here's how I fixed it for my situation. You are correct, changing the font size for code view seems to leave the actual printed code untouched (in 12pt monaco --- huge). I went to File->Page Setup and actually scaled down the entire page (in my case I took it down to 50%, and when I printed the code, it worked --- reducing the code to 6pt).
