I'm not sure what path these features follow exactly, but it seems more or less based on the order the objects were created. If there are a lot of disassociated text blocks pasted here and there, that's how Spell Check will behave.
Edit: Just did a few tests to confirm that spell check does follow the order in which the text frames were created. It doesn't matter whether copied and pasted from another frame or created new, it is the order of creation of the frame that is followed.
If randomly created text frames are threaded to follow page by page through the document, spell check will follow the new threading. If randomly created frames are cut and pasted in order of how they appear, Spell Check will follow the new order.

carl_hart Apr 30, 2011 12:16 PM (in response to M Blackburn)Thanks very much. That makes perfect sense. They are indeed disassociated and not created sequentially. I wish there were a feature which would override this and just follow the page order. I'll have figure out how threading works.
