    Index. Pages don't show


      I'm trying to work with the Index function.  When I "tag" something to appear in the index, I highlight what I want (e.g. "New York") in the index, use "Create a new index entry" and get a window showing me "New York" in the Topic Levels #1 box, then in Type I select "Suppress Page Range" because I want it to look through the entire document, then "Add All".  This gives me a listing in the Index window for "New York" including all the pages where it appears.  Two problems.  First, if "New York" appears twice on the same page, in the Index window twice.  Second problem is that when I select "Generate Index" it gives me a listing for "New York" but no page numbers whatsoever.  Your help is appreciated.
