Never mind. The issue was so simple I feel stupid. The "Print to File" check box - that small little box at the bottom of the print window, was checked.
I have had issues in the past were acrobat and reader fight for control. Could this be the case here?
As long as you are running version X of both, it is now ok.
Check the print dialog for "Print to file". Deselect it.
The cover page of the following document is unprintable. You get an error message saying "no pages selected" if you start with page 1. You can only print the report if you avoid page 1 and start on page 2. I do not have problems with any other PDF documents.
I'm using Windows XP and a Laserjet 1000. This problem occurs with both Adobe Reader X or Foxit Reader 126.96.36.19905

So it must be something peculiar about the way the cover page is designed.
