Without knowing what options you exactly used in Photoshop its a little hard to tell what may have gone wrong.
Couple of things to think about:
- When you modify a JPEG file you uncompress it and then recompress it using a lossy JPEG compression again which reduce the fidelity of the image
- Try outputting to a PNG
- Did you have downsampling turned on when you made it smaller? That can also make is lower quality.
- Photoshop has a lot of options so you may had something set that reduced the quality - look at the options.
- Try resizing it using another program - resizing can be done in MS Paint or other basic editing program

We're about to release some questionaires to customers and would linke to use non compressed images - How is the investigation coming along?

A workaround like linking images doesn't work eighter, because formcentral embedds all images.
