There have been numerous issues since CS-5 has come out with all the hardware I/O cards. Hopefully with the release of 5.5.1 things will get better. Looking at the Blackmagic page, their last Decklink driver release was 8-29-11. This clearly couldnt address the Adobe 5.5.1 release yesterday. Give them a little bit of time to get it rolled out.
Adobe is clearly are aware of driver issues and posted this with the 5.5.1 update:
We have also been working with several providers of plug-ins, codecs, and hardware devices (such as Cineform, AJA, Nvidia, and BlackMagic) to assist them in updating their software to fix some errors and crashes. Please take this opportunity to download and install updated codecs, plug-ins, and drivers from these providers, as relevant to your work.
I had a similar problem with a Matrox card. The solution was to turn on desktop composition. It is under system properties - advanced TAB, performance - about the 4th in the list. Some programs (AVID for example) insist on this being turned off, but with CS5.5 I needed it enabled. I have not tested this with Black Magic devices. It did not cause me a problem with CS5,
I´d love to try this but I can´t seem to find that desktop composition option in my system. Where is it? Windows properties? Nvidia? Premiere?

You need to go to the control panel in Windows, choose system (which you can also get to by going to the my computer icon, right clicking and choosing properties.
On the left hand side choose ADVANCE SYSTEM SETTINGS
You get a box with 5 TABS - the third one says Advanced
The top box on this tab says performance
The forth item down is ENABLE DESKTOP COMPOSITION. In the above picture it is off. Tick it to turn it on. Then obviously apply and ok to get rid of all the boxes.
Do this with Premiere shut. You should not need to reboot just restart Premiere after you have done this and see what happens.

Like I said this worked on a Matrox system, I have not tried it on a Black Magic but hopefully it is the same thing.
Thx David, I did not seem to find it because my OS being in spanish, silly me.
Anyway, I tried but no fix. Everytime I create a Premiere Pro preset timeline (a 25p 1080 DSLR one) my monitor goes black for a second (like changing mode to something) and then comes back to life but the Blackmagic timeline can´t be seen anymore. It is like if creating the Premiere Pro tiemline kills the layer where the Blackmagic video should be.
I will wait for new drivers I guess.
Sorry to hear that did not work. If I get a chance I will try it out on a Black Magic card I have her and see what happens. My Black Magic card is currently in a system which does not have the Mercury playback engine going because I find I get better performance with Black Magic with the MPE off than on.
Well, finally got it working! It looks like I can´t select only the desktop composition only, I have to choose the last 2 options too, or it would not activate the desktop composition option.
