you can add fields in Acrobat.
I suggest you do some reasearch on PDF and forms as there is LOADS of info and techniques... also check out : theindesigner.com I think he has a nice tut on this.

I already tried to add fields directly in Acrobat. But the problem is the fields should be at very specific coordinate. It would be necessary to transfer the guides from Indesign to Acrobat Pro, but I think there's no way to do that.

yes there is: export pdf: under general include visible guides and grids...

This helped me a lot. But when I include visible guides they become printable.
I would like to know if there is a way to use the guides of InDesign but make them invisible for print.
Put them on their own layer in ID and make the layer visible but nonprintable.
