Can you generate a PDF file directly from your scanner? Or, do you need to find a product that generates a PDF file from something like a JPEG file?
Once you have a PDF file, then you should be able to use Adobe Reader to do what you're asking. The latest version of Adobe Reader (10.1.2) allows you to add text, sign and save directly on the PDF file. It is available via the Sign pane on the right side panel. Check it out!

To make a fillable form form a scanned image, you need Adobe Acrobat X Professional. You can scan your document to PDF using Acrobat, then use the Form Wizard function (or manually) add form fields to the appropriate places on your document. Once form fields have been added, you can then fill out the form using either Adobe Acrobat or Adobe Reader.
FormsCentral is used to create HTML-based forms and isn't designed to work within this workflow at this time.
