Table of Content - items not properly ordered

I'm using in TOC the following paragraph styles: Chapter, Title and Author which are Level 1, 2 and 3.

But because some Articles don't have any authors, it messes up with the proper order

      Chapter 1

           Title 1

           Title 2

      Chapter 2

                Author from Chapter 1/Title 2

           Title 1

           Title 2

If anyone has any idea how to repair this, it'll be greatly appreciated.

I also have another question related to TOC (though not so important right now)..how can I update it when it spreads through multiple frames?

Now I delete all the frames till it remains only one for the Update TOC to be available.

      Thank you.

In general, out of order TOC entries are caused by element positioning on the page. ID builds a TOC by starting in the upper left corner of your page, then reading down before moving right and reading down from the top again. If you post a screen shot of the page(s) we might be able to offer some suggestions.
